Add 1 cup of half-and-half along with chopped chocolate into a small pan over low heat, stirring just until chocolate is melted. Pour into a bowl and mix in remaining half-and-half and sweetened condensed milk, whisking until well combined.
Cover and refrigerate 2 hours.
Pour into ice cream maker and follow manufacturer’s directions. Drop in caramel by 1/4 teaspoonfuls and stir to mix in. Place in an airtight container and freeze until ready to serve.
Remove from freezer about 10 minutes before ready to serve to allow ice cream to soften slightly.
